Mission
The mission of the Forms Committee is to create uniform,
simplified, and user-friendly forms and instructions for adoption by the
Judicial Council to facilitate excellent communication between the public, the
local departments of child support services, and the courts.

Goals
The Forms committee:

Meets periodically to review, modify, and propose family law and governmental
forms for adoption or approval by the Judicial Council

Keeps the Child Support Directors Association directors informed of the status
of forms under review

Distributes newly approved or adopted forms as soon as possible for
implementation by the six interim child support automation systems
